We currently have an opportunity for a Chronic Disease Nurse, Diabetes in our Chronic Disease Management Program located at Abbotsford Regional Hospital & Cancer Centre in Abbotsford, British Columbia.
This position offers you the opportunity to:
• Participate in planning, implementation development and evaluation of clients
• Support patient in self-management of chronic diseases
• Promote and assist with healthy lifestyle
• Collaborate with health care team members
To be the successful candidate, you will have:
• Graduated from an approved school of Nursing
• Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
• Recent 2 years’ experience assessing and treating patients with chronic health needs
• Accreditation as a Certified Diabetes Educator (CDE)
• Current CPR certification
