Personal Support Worker (PSW)

PURPOSE AND SUMMARY –

Personal Support Worker is a multi-functional professional who is responsible for the delivery of in home support and personal care to ill, frail, handicapped and disabled persons of all ages so that they may remain independent in their own homes and communities for as long as possible.

DUTIES AND RESPONSIBILITIES-

• Providing assistance with personal care activities such as but not limited to bathing, dressing, transferring, walking, handling stairs, dressing, and grooming; assisting with bedside care as required;
• Providing assistance with routine activities of daily living and supporting the safety of clients and self;
• Assisting with the use and monitoring of special aids (i.e., walkers, cranes, wheelchairs, lifts, crutches, etc.)
• Recognizes and reports changes in a client’s behaviour/ health condition.
• Provides informal counselling, as needed.
• Following basic hygienic practices and applying the principles of body mechanics;
• Light Housekeeping Duties: Common Areas, Kitchen, Bathrooms, bedrooms…Laundry
• Transportation Services – Escorting to personal/medical appointments as needed
• Assisting with medication monitoring according to policies and procedures of the agency;
• Assisting with meal planning and shopping, planning and preparation of nutritious meals, including special diets;
• Providing ongoing support to family members involved in client care; encouraging involvement in client care and facilitating effective and ongoing communication;within a multidisciplinary team approach, assisting with support and training of primary caregivers in the care of infants and children according to the Care Plan;
• Providing childcare and support where required;
• In cases where palliative care is necessary, offering sensitive physical, mental and emotional support; providing assistance with activities of daily living as identified in the care plan;
• Understanding his/her role and responsibility in maintaining a safe workplace and reducing workplace injuries;
• Ensuring accuracy, confidentiality and safekeeping of agency records

QUALIFICATIONS

Education a minimum one-year PSW Certificate is required.

• Work Experience – a minimum of (1) year direct experience in the provision of in home personal care and/or community based home care service delivery; – experience in the facilitation and/or coordination of community programs and services is an asset.    We welcome new grads!

• Proven ability to effectively and safely provide personal care, personal support, palliative care and respite to elderly, disabled, and special needs children within a home setting;
• Excellent time management, organizational and coordination skills;
• Good written and oral communication skills;
• Excellent interpersonal skills,
• Ability to effectively problem-solve;
• Ability to maintain confidentiality;
• Ability to follow direction and work independently, exercise good judgment, and prioritize daily tasks;
• Ability to work effectively and sensitively with children and families;
• Demonstrated understanding of the needs of the elderly, ill, physically challenged, and other vulnerable individuals and families;
• Knowledge of gerontology, community health, and multi-disciplinary programming as it relates to the physical, spiritual and psychosocial needs of the target population;
• Knowledge of current issues affecting the home health care needs
• All staff must communicate verbally and in writing in English or other languages welcome. Must be able to handle difficult situations, work independently, & as part of a team, able to perform and prioritize multiple functions/tasks; able to stand, bend, squat, kneel & reach freely; able to freely lift 23 kilograms; & able to travel locally from assignment to assignment.

CONDITIONS OF EMPLOYMENT

1. Must have a Personal support Worker Certificate or be willing and able to complete Personal Support Worker Certificate training;
2. He/she must demonstrate the willingness and ability to work with clients of all ages, including infants, children, adults and the elderly;
3. He/she must possess a Standard First Aid and CPR Certificate/Mask Fit Test or be willing to obtain one;
4. He/she must be able to work flexible hours in a home, school, and/or community setting as required;
5. He/she must have a valid Ontario driver’s license and access to a personal vehicle;
6. A criminal record check is required upon hiring.
