Ultimately, this is where you want to be.POSITION SUMMARYThe patient safety specialist will be reporting to the Manager of the Quality & Patient Safety Patient Safety and responsible for the day-to-day general operations management within the Patient Safety program as assigned.The Patient Safety Specialist is responsible foster a Culture of Safety, adopt methods to identify and evaluate risks for harm and to mitigate and manage risk for harm. The patient safety specialist will:Cultivate a positive, safe healthcare environment by role-modeling, promoting safe practices, nurturing a just culture, incorporating systems thinking, and improving evidence -based behaviour and practices that improve patient care to reduce harm throughout the entire healthcare system.Design and implement standardized safety enhancement strategies, methods, and tools to systematically assess potential or actual risk throughout the health system or organization to promote and sustain safety outcomes.Provide oversight to the coordination of actions taken to lessen or eliminate a risk, thereby reducing the likelihood of occurrence/ recurrence, or prevent and reduce the level of harm should the risk occur/ reoccur. This includes a plan for implementing interventional strategies and a process for monitoring progress and effectivenessPRIMARY RESPONSIBILITIESPromote a safety culture and infrastructure that aligns with the organization’s strategic objectivesCultivate a culture of continuous learningAdvance patient / person and family-centred strategies that enhance safetyIntegrate human-factors principles within the organization’s patient safety frameworkDevelop a systematic process that identifies and reports patient safety risks, near misses, and adverse eventsUse methods, tools, and technologies to drive a causal analysis programApply techniques to interpret and communicate finding of potential or identified risksIdentify the active and inactive conditions within the work environment that contribute to the specific area(s) of risk / defect.Integrate best practices and evidence – based mitigation strategiesDetermine appropriate actions for mitigation or management of identified risks, defects, and root causes.QUALIFICATIONSBachelor’s degree in a professional health discipline, engineering or a related field, from an accredited university.Master’s Degree in Quality Improvement Patient Safety or Health Administration is preferred.Institution for Healthcare Improvement (IHI) patient safety certificate, Certified professional in Health Safety (CPPS), Certified Professional Healthcare Quality (CPHQ), Canadian Patient Safety Institute (CPSI) Officer certificate or equivalent is preferred.Lean Green and/or Black belt or equivalent preferred.Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is preferred.Willingness to actively pursue certification in Patient Safety, if not already attained.Minimum 2 years working experience in healthcare patient safety, healthcare risk management, quality or professional practice.Recent experience with project and change management is preferred.Well-developed public speaking abilities and exemplary written and verbal communication skills, with the ability to establish positive collaborative working relationships with all staff, clients, families, community agencies and other professionals while employing tact and diplomacy.Dynamic, innovative and creative leadership, especially in the development of, and delivery of, education material to adult learners; interpersonal and team building skills to engage staff.Proven organizational, problem solving, time management, prioritization, coordination, facilitation, and critical thinking abilities.Detail oriented, objective, and demonstrated commitment to just culture.Ability to handle challenging and sensitive situations with professionalism, tact, diplomacy, objectivity, and in discreet confidence.Highly computer literate, including working knowledge of Microsoft Office Suite (Outlook, Word, Power Point, Excel, and Access).Proven attendance record.Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.Ability to perform the essential duties of the job.Effectively uses empathy in interactions with others.Demonstrates behaviours consistent with Mackenzie Health’s Commitment to Caring.Models behaviour that is aligned with the values of Mackenzie Health – Excellence, Leadership and Empathy.Commitment to providing an exceptional experience for staff and clients within a challenging and exciting health care environment.Mackenzie Health is an inclusive and equal opportunity employer committed to providing accommodations for applicants upon request at any stage of the recruitment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) and the Ontario Human Rights Code.
