Ultimately, this is where you want to be.The Patient Care Coordinator is an important member of the Medical Imaging leadership team and is responsible for the overall day-to-day department-based coordination of patient and family care, facilitation of patient flow in and out of Medical Imaging, coordination of technical and clerical staff and resources, and conduct of supervisory support functions such as attendance and performance management, and conflict resolution at the point of care and beyond. Contribute to safe, quality care by• Provides oversight of clinical Care Plan and use of Quality Based Practices (QBP) Pathways in Medical Imaging.• Leads inter-professional Rounds and use other tools to support coordination of care and Patient Flow.• Ensures completion of high risk assessment for discharge and delegates the discharge planning as appropriate.• Provides support to staff for issues related to patient care and families.What must you have?• Member in good standing of the College of Medical Radiation Technologists of Ontario (CMRTO).• Baccalaureate Degree or current enrollment in such a program.• Minimum of five (5) years of relevant clinical care experience.• Demonstrated leadership skills with a minimum of two (2) years of supervisory experience.• Current Basic Cardiac Life Support (BCLS) certificate.What else do you bring?• Knowledge of CMRTO standards of practice and a clear understanding of the application of the RHPA legislation.• Ability to work independently, self-directed and can effectively lead clinical inter-professional teams.• Highly developed conflict resolution skills.• Well-developed ability to organize daily departmental activities and priorities.• Ability to demonstrate commitment to professional development and learning.• Excellent interpersonal, communication, coaching, mentoring and team building skills that facilitate positive interactions among all members of the health care team.• Strong understanding of continuous improvement principles and tools.• Highly developed analytical, problem solving, critical thinking skills.• Strong patient and customer focused philosophy in all interactions.• Ability to introduce and effectively facilitate change.• Demonstrated ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations.• Effective listening skills to maximize understanding of issues.• Proven attendance record.• Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.• Ability to perform the essential duties of the job.• Effectively uses empathy in interactions with others.• Demonstrates behaviours consistent with Mackenzie Health’s Commitment to Caring.• Models behaviour that is aligned with the values of Mackenzie Health – Excellence, Leadership and Empathy.• Commitment to providing an exceptional experience for staff and clients within a challenging and exciting health care environment.Preferably, your profile also includes:• Membership in Ontario Association of Medical Radiation Technologists (OAMRT) is an asset.• Membership in Canadian Association of Medical Radiation Technologists (CAMRT) is an asset.*May be required to work at any site of Mackenzie Health
