Overview:
The Nurse Practitioner that joins MEST has the opportunity to
utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for COVID-19 (+) residents in LTC and congregate settings. In collaboration with the MEST Medical Director, the LTC interdisciplinary team and resident families, The Nurse Practitioner is expected to provide a comprehensive health assessment and diagnosis of medical problems as related to COVID-19 (+) residents. This may involve the development and implementation of a resident plan of care, as well as the provision of health counseling and emotional support in the areas of illness and injury prevention, complications, and health promotion. The Nurse Practitioner will
support the interdisciplinary teams to optimize the COVID-19 (+) resident’s treatment plan, to improve the health outcomes of those residents diagnosed with COVID-19 and ideally reduce admission/readmission to hospital.
Unit Profile:
The COVID-19 pandemic has deeply affected long-term care facilities around the world. In response to this global crisis and to the needs of the vulnerable residents with-in LTC and congregate settings in our surrounding communities, Halton Healthcare has developed a Rapid Support Team of clinicians, Allied Healthcare Professionals and Pandemic Support Leaders that have united to form MEST (Mobile Enhancement Support Team.) This team of dedicated individuals is deployed to different long-term care facilities (or congregate settings) who are experiencing different crisis situations related to COVID-19. This mobile support team reviews the individual needs of each LTC facility that has been identified as “in crisis” and together develops a customized support plan addressing the main challenges the facility is facing.
Ideal candidates are strong in resident centered care, able to work in a dynamic, rapidly changing environment, are ready to embrace change and innovation and are able to travel with-in Halton and neighboring regions. Ideal candidates will see the long-term care facility as the resident’s primary home and understand that it is operated as a place where the resident lives with autonomy and dignity. The MEST NP role requires and individual with excellent clinical and communication skills, a clear resident and resident family focused approach.
Additional Responsibilities may include:
Direct Comprehensive Care:
Obtains and documents a comprehensive health history, performs a physical examination on COVID (+) residents requiring assessment, collaborates closely with the MEST Medical Director and facility physician/Nurse Practitioner team and other members of the LTC interdisciplinary team. Furthermore, the NP has a role for advancing a resident’s plan of care where necessary and working with the MEST Medical Director and LTC interdisciplinary team to improve the COVID-19 (+) residents outcomes and reduce admission to hospital.

Halton Healthcare Services is seeking individuals who are leaders in their chosen profession, patient/customer-centered, team players, effective communicators, caring and respectful and are fiscally responsible.
Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario required.
Completion of a Master Degree in Nursing required.
Eligibility for cross-appointment to an academic institution preferred.
Membership in specialty related organizations e.g. NPAO, RNAO.
Minimum of 5 years nursing experience in a general internal medicine required.
Recent experience as an NP in General Internal Medicine preferred with experience in LTC an asset.
Innovative, creative and professional role model.
Demonstrated ability to prioritize/organize multiple demands.
Ability to work autonomously.
Proficient with Microsoft Office applications including Outlook, Word, Excel, PowerPoint and Visio required.
Proficiency with the Point Click Care LTC documentation software is an asset.
Ability to meet the physical demands of the position.
Satisfactory employment and attendance record required.
Ability to work as a patient facing clinician with COVID (+) residents.
Ability to travel with-in Halton and neighbouring regions.
Responsive time of 48 hours to 72 hours when the MEST team is redeployed to a new LTC facility.
Salary:
$50.51 to $61.78 per hour
