Hours of work are Monday to Friday 9 am to 5 pm and the tentative start date is April 19, 2021.What will you do? In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plansLink patients with service providersCoordinate and monitor care plan deliveryEstablish a helping relationship with patients and their familiesBalance patient needs and choices with available resources, ensuring patients’ values and preferences are respectedWhat must you have?Registration as a health or social work professional, including registered nurse, physiotherapist, occupational therapist, speech language pathologist, or social workerMembership, in good standing, with the applicable regulatory body:College of Nurses of OntarioCollege of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioCollege of Audiologists and Speech Language Pathologists of OntarioOntario College of Social Workers and Social Service Workers2+years of recent experience in community health or a related fieldKnowledge of the health care delivery system and community resourcesExcellent interpersonal, communication, assessment and decision-making skillsEffective time management skills, with the ability to work independently and co-operatively in a busy multidisciplinary environmentA valid driver’s licence and access to a reliable vehicleAbility to use a computer in a Windows environmentWhat would give you the edge?A university degree preferred (or an equivalent combination of education and experience may be considered)Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatricsAbility to speak French or another second languageWho we areLocal Health Integration Networks (LHINs) play a key role in Ontario’s health care system.
