In this position you will be responsible for providing patient-focused care across programs at TRI which include spinal cord rehab at Lyndhurst Centre;
complex continuing care, palliative care, long term low duration rehab and special care unit at Bickle Centre; geriatric rehab, dialysis, neuro rehab, and musculoskeletal rehab at University Centre.
You will work closely with the inter-professional teams and pharmacists and pharmacy technicians to optimize medication use and safety through the provision of pharmaceutical care and to ensure the safe delivery and administration of medications to patients.
For this role, we are looking for highly motivated, team-oriented individuals with sound knowledge in the provision of pharmaceutical care.
The pharmacist is expected to have superior inter-professional, communication and organizational skills and demonstrated teamwork in a patient care setting.
In this position the pharmacist’s primary responsibilities include: conduct medication reconciliation, review and dispense all doctors’ medication orders for appropriateness; identify, resolve and prevent drug therapy problems with the patient and health care team; monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines; follow-up on medication incidents and promote safe medication practices on the unit; respond to drug information questions; patient education; facilitate discharge reconciliation; provide direct and/or functional supervision of technician staff.
In addition, the pharmacist is expected to support clinical teaching and research activities within the department while exemplifying UHN’s values of caring, excellence, teamwork, innovation, integrity, leadership, and respect.
Possess a Bachelor’s of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required.
Certificate of competency from the Pharmacy Examination Board of Canada required.
Active registration with the Ontario College of Pharmacists (Part A) required.
Hospital pharmacy practice experience preferred.
Hospital Pharmacy residency an asset.
Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset.
Excellent verbal and written communication skills.
Excellent customer service & interpersonal skills.
Excellent organizational and time management skills.
