Sponsorship Eligibility Expires:2019-05-14 Responsibilities/Duties: The Specialized Geriatric Services Program (SGS – Clinical Alternative Relationship Plan) in partnership with Alberta Health Services is inviting applications for a Geriatrician / Care of the Elderly Physician to join the Divisions of Geriatric Medicine / Care of the Elderly located in Edmonton, Alberta, Canada. Alberta Health Services is Canada’s largest and only provincial health region providing services for a population of over 3 million people. Qualifications: The successful candidate will be accredited in either General (Internal) Medicine and Geriatric Medicine or Family Medicine with a Care of the Elderly certification (MD with equivalent qualification) and eligible for registration with the College of Physicians and Surgeons of Alberta.This post is an unrivalled opportunity for an interested, engaged and enthusiastic physician to be involved in the development of a new senior’s service and model of care in the community including that provided by Edmonton’s Northeast Community Health Centre and North Homecare Network.The successful candidate will spend 5 (0.5 FTE) sessions per week providing ambulatory care services to the Northeast Community Health Centre in North East Edmonton. The other 5 (0.5 FTE) sessions will be served in Homecare, with the primary site being the Northeast Homecare Network (located centrally on Kingsway Ave). The physician will provide cross coverage to other network offices when needed.North East Community Health Centre: The physician will be integral to the consultation process and subsequent development of the ambulatory service at Northeast Community Health Centre. The post holder will see local referrals and newly referred patients from the Edmonton Zone Central Access System, providing much needed support to local primary care practitioners and serving the local community.Homecare Northeast Network: Duties will include assessment and management of frail, medically complex older people in receipt of home care services, home based assessments and targeted interventions for those people either deteriorating in the community or following an acute or rehabilitative admission. Referrals will be received either from home care, from local primary care providers or from facility based physicians or transition care coordinators. The physician will be supported by the homecare multi-professional team and accountable to the Medical Lead in Homeliving. The post holder will have the opportunity to facilitate the engagement of primary care and consider innovative collaborations with local primary care networks.The post holder will be expected to participate in the undergraduate and postgraduate education and training programs of the joint divisions of Care of the Elderly and Geriatric Medicine and maintain adequate continuing professional development as dictated by their relevant governing body. Engagement with and participation in quality improvement and assurance activities is expected. Medical liability insurance will be required Compensation: All appropriate support (administrative and technical) to enable the successful candidate to excel will be provided. Remuneration for the post is $327,468 – 359,832 depending upon training and hours.
