We are looking for candidates with a background in long-term care and geriatric care and with a desire to make a difference in the lives of our residents. The Documentation Nurse is responsible for the ongoing implementation and maintenance of the resident electronic plan of care in consultation with the multidisciplinary team at the Home. They are also responsible for assisting in the planning, directing, and implementing activities that support the efficiencies of the nursing care team and the safe and effective delivery of care to residents. This person participates as a member of the RAI-MDS team of the home under the direct supervision of the RAI/Quality Coordinator. RESPONSIBILITIES Functions in accordance with related College of Nurses of Ontario Standards of Practice; Resident Care Policies; MOHLTC Standards of Resident Care and other related guiding principles and standards within the LTC Home. Supports the MDS process. Communicates tactfully and courteously with residents, families, staff, and visitors. Establishes and maintains good working relationships with all personnel in the Home. Works in conjunction with Restorative Care Team to ensure resident outcomes are maximized. Performs nursing care for residents as required, including supporting RAI and Point Click Care clinical documentation requirements. Participates in and/or monitors the admission and discharge planning process. Communicates policies and procedure changes to staff as required Maintains RAI coding competency as required. Updates Care Plans and maintains RAI assessments in the required time frames in an accurate manner. Performs other duties as assigned.
