The community is at the end of a long, truly magnificent fjord, on the mainland coast of British Columbia. Bella Coola General Hospital has 10 acute bed, 5 residential beds and a 3 bed emergency department open 24/7, along with a laboratory, diagnostic imaging services, Telehealth, Mental Health, Public Health, Home & Community Support Services. Also within the hospital is a medical clinic and pharmacy.JOB SUMMARY:Within the context of a client and family centered care model and in accordance with established vision and value of the organization, the Community Health Worker 2 works under the direction of the Supervisor/Coordinator/Manager or delegate to provide home support services to clients.Assists clients with the activities of daily living within varied settings to promote maximum independence, observes and documents on client’s health condition and environment, reports significant and unsafe observations and provides support and guidance.Provides input and feedback in the care planning process, assists in meeting the nutritional and elimination needs of clients, and performs delegated tasks under the Transfer of Function guidelines for clients in specific situations for which training has been provided.QUALIFICATIONS:Education & ExperienceGrade 10 plus graduation from a recognized Home Support/Resident Care Aide program.Valid Class 5 BC Drivers License.Local travel requires the use of a personal vehicle.Knowledge, Skills & AbilitiesHome management skills.Physical ability to carry out duties of the positionAbility to work independently and in cooperation with othersAbility to operate required equipmentAbility to communicate effectively both verbally and in writingAbility to organize and prioritizeAbility to observe and recognize changes in clientsAbility to establish and maintain rapport with clientsThe hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
