Temporary Full-Time Care Coordinatorpositions are currently available at Toronto Central Community Local Health Integration Network (TC LHIN). Specifically these roles are to work with our major hospital providers in the downtown core (ie. University Health Network, Mount Sinai, St. Michael”s). This opportunity is great for regulated health professionals who want to help clients transition from Hospital to Community with safety and ease.POSITION SUMMARYIncumbents will assess referred clients for eligibility in the Toronto Central Local Health Integration Network and assist ineligible clients in finding alternative sources of care.
